- Warfarin inhibits Vitamin K epoxide reductase (VKOR), blocking regeneration of reduced Vitamin K, thereby impairing hepatic synthesis of clotting factors II, VII, IX, and X, as well as Protein C and Protein S.
- Factor VII has the shortest half-life, making the PT/INR the first to rise after warfarin initiation and the primary monitoring parameter.
- Protein C and Protein S are anticoagulant factors also inhibited by warfarin; their rapid depletion early in therapy can cause a transient hypercoagulable state, explaining warfarin-induced skin necrosis (especially in Protein C deficiency).
- Warfarin crosses the placenta and is teratogenic (fetal warfarin syndrome: stippled epiphyses, nasal hypoplasia); heparin is the safe anticoagulant alternative in pregnancy.
- Cytochrome P450 interactions are high-yield: enzyme inducers (rifampin, carbamazepine, phenytoin) decrease warfarin effect; enzyme inhibitors (fluconazole, amiodarone, metronidazole) increase warfarin effect and bleeding risk.
- For supratherapeutic INR without serious bleeding, withholding warfarin ± oral Vitamin K1 (phytonadione) is appropriate; onset of INR correction with oral Vitamin K takes 24–48 hours.
- For life-threatening or intracranial bleeding, immediate reversal requires 4-factor Prothrombin Complex Concentrate (4F-PCC) plus IV Vitamin K; fresh frozen plasma (FFP) is an alternative but carries volume overload risk.
- Warfarin effect is potentiated by Vitamin K-deficient states (malnutrition, prolonged antibiotic use eliminating gut flora) because less dietary substrate competes with warfarin's mechanism.
- Therapeutic INR goal is 2–3 for most indications (DVT/PE, atrial fibrillation, bioprosthetic valves); INR goal is 2.5–3.5 for mechanical mitral valves or recurrent thromboembolism on therapy.
- Heparin bridging is used when initiating warfarin for acute thrombosis because warfarin takes 5–7 days to achieve full anticoagulation, and early Protein C/S depletion creates initial hypercoagulability.
