- Aliskiren is the only FDA-approved direct renin inhibitor, binding the active site of renin to block conversion of angiotensinogen to angiotensin I.
- By inhibiting renin directly, aliskiren reduces plasma renin activity (PRA) while causing a compensatory rise in plasma renin concentration (PRC) due to loss of negative feedback.
- Aliskiren is indicated for hypertension but is contraindicated in combination with ACE inhibitors or ARBs in patients with diabetes or CKD (GFR <60) due to increased risk of hyperkalemia, hypotension, and renal impairment.
- Aliskiren is absolutely contraindicated in pregnancy (Category X/teratogenic) due to fetal renal dysgenesis, hypotension, and death — same black-box warning shared with ACE inhibitors and ARBs.
- Unlike ACE inhibitors, aliskiren does not cause bradykinin accumulation and therefore does not cause a dry cough or angioedema via that mechanism.

- Aliskiren has poor oral bioavailability (~2.5%) and a long half-life (~40 hours), allowing once-daily dosing.
- High-fat meals significantly reduce aliskiren absorption; patients should take it consistently with or without food.
- Aliskiren is a substrate and inhibitor of P-glycoprotein; concurrent use with cyclosporine or itraconazole is contraindicated due to markedly increased aliskiren levels.
- In heart failure and post-MI patients already on ACE inhibitors or ARBs, adding aliskiren showed no benefit and increased harm (ALTITUDE trial), reinforcing its contraindication in dual RAAS blockade.
